Employment Details to be Provided by the Applicant
Apart from personal data, an applicant should also provide all the information about his previous employment in noisy occupations:
- the name, contact address and telephone number of the employers of each noisy occupation;
- the period of employment, post, earnings and details relating to the use of noisy tools/machines for each noisy occupation; and
- for each noisy occupation, the contact addresses and telephone numbers of two co-workers who know about the details of applicant's employment in that job.
Information about Co-workers
In verifying an applicant's employment, the Board may need to contact his co-workers to clarify the details. If the applicant can provide Information about the contact addresses and telephone numbers of his co-workers, it would facilitate the processing of his application by the Board.
Supporting Documents
Applicants should, as far as possible, provide any documents which substantiate their employment in noisy occupations. Some examples are:
- letters issued by employers certifying the dates of employment, post and earnings;
- employment contracts;
- notification on the computation of severance payment or termination payments;
- salary tax returns filed by employers; and
- wage receipts, etc.
For the purpose of determining the amount of compensation, an applicant is also requested to provide documentary evidence on his earnings in respect of his last 12 months' employment in specified noisy occupations prior to his application for compensation. If the applicant cannot provide such evidence or the evidence so provided is not accepted by the Board, the amount of compensation payment will be calculated with reference to the median employment earnings of the total employed population of Hong Kong published by the Census and Statistics Department.
